Suppression is the piece of strabismus and amblyopia that doesn't show up on a cover test, isn't visible to family or friends, and often isn't addressed at all by treatment focused purely on alignment or acuity — which makes it the most commonly missed part of full functional recovery.
What Suppression Actually Is
When the two eyes send the brain conflicting or poorly matched images, the brain actively suppresses input from one eye to avoid double vision or visual confusion. This is an adaptive, functional response, not a malfunction — it's the brain solving a real problem with the tools available to it. In strabismus and amblyopia, though, this adaptive mechanism generalizes further than it needs to, suppressing input even for visual information that could otherwise be fused, and that overgeneralized suppression is what quietly limits functional recovery.
Why Alignment Alone Doesn't Fix It
Strabismus surgery corrects the physical position of the eyes. It doesn't retrain the brain's suppression pattern, which developed over months or years and doesn't reverse itself automatically just because the eyes are now pointed in the same direction. A well-aligned eye can still be functionally suppressed — meaning the surgery succeeded by every visible measure while the underlying binocular dysfunction remains untouched.
Why Patching Alone Doesn't Fully Fix It Either
Traditional patching for amblyopia forces the weaker eye to work, which can improve acuity, but it does nothing to address the brain's suppression mechanism directly — if anything, it trains monocular function rather than binocular cooperation. This is a large part of why patching gains regress once the patch comes off: the suppression pattern that was there before treatment is often still there underneath the improved acuity.
What Actually Addresses Suppression
Binocular and dichoptic approaches — activities and technologies that present different, carefully balanced information to each eye simultaneously — target suppression directly, gradually rebalancing the competition between the eyes rather than just strengthening one side. This is the mechanism behind why modern binocular vision therapy tends to produce more durable results than approaches that only address alignment or acuity in isolation: it's treating the actual hidden problem instead of one of its downstream symptoms.
