A clean MRI after a concussion tells you something important, but it doesn't tell you everything — and understanding exactly what it does and doesn't rule out changes how you should interpret a normal result.

What Structural Imaging Is Actually Built to Find

MRI and CT are designed to detect structural damage: bleeding, a visible lesion, a mass. That's a genuinely important thing to rule out, and a clean scan means the brain doesn't have that kind of damage. What it was never built to detect is functional performance — how efficiently the accommodative, vergence, and oculomotor systems are actually operating day to day. Structural integrity and functional performance are different questions, answered by different tools.

What Functional Imaging Has Shown Instead

Research using functional MRI, a different technique from the structural imaging used in a standard clinical workup, has found measurably abnormal activation patterns in the brain regions that control eye movement in concussed patients — even in patients whose standard structural imaging came back completely clean. That gap between a normal structural scan and a real, measurable functional deficit is the actual explanation for why so many patients get told everything looks fine while they continue to struggle.

What Testing Actually Catches This

Near point of convergence, accommodative facility, MEM retinoscopy, and standardized oculomotor testing (DEM, NSUCO) directly measure the functional performance a structural scan can't see. Stereoacuity, vergence facility, and full vergence range testing add the binocular coordination piece, while fixation disparity and/or subjective phoria testing, visual midline shift testing, and VO star testing round out the picture of how the visual system is actually functioning under real demand. These aren't exotic tests — they're standard developmental optometry tools, applied here to a population whose deficits are real but invisible to the imaging most providers already ordered.

What to Do With a Normal MRI Result

A clean MRI is genuinely good news about structural injury. It's not a reason to stop looking for an explanation if symptoms persist. If a normal MRI, a clean general neurological exam, and ongoing symptoms describe your situation, the next reasonable step is functional testing of the visual system specifically — the piece of the workup most standard concussion protocols never get to.
