On paper, these two conditions can look nearly identical: blur at near, fatigue, headaches, avoidance of reading. In the exam chair, they're two different systems, and mixing them up leads to a treatment plan that only addresses half the problem.

Two Different Jobs

Accommodation is the lens focusing at different distances. Convergence is the two eyes aiming together at a single point. They're neurologically linked — the AC/A and CA/C relationships mean effort in one system drives some reflexive response in the other — which is precisely why a patient's own description of their symptoms can't reliably tell you which system is actually failing.

What Isolated Testing Actually Separates

Accommodative facility, tested with plus/minus lens flippers, and MEM retinoscopy characterize the focusing system specifically. Vergence facility, tested with prism flippers, and fusional vergence ranges characterize the binocular coordination system specifically. Running both, rather than assuming one from a clean-looking result on the other, is what actually differentiates these two conditions — a patient can fail one, the other, or both, and each combination changes the treatment plan.

The Overlap Trap

Because the two systems are neurologically coupled, a patient with primarily accommodative dysfunction can show mildly reduced vergence findings as a secondary effect, and vice versa. This is where isolated testing earns its place: it tells you which system is the primary driver and which findings are downstream of that primary problem, rather than treating every abnormal finding as an independent condition needing its own separate treatment plan.

Why Getting This Right Changes the Treatment Plan

Accommodative dysfunction responds to accommodative-specific management — lens power, accommodative facility training. Convergence insufficiency responds to vergence-specific therapy. A mixed presentation, which is common, needs both addressed in the same plan rather than sequentially. Treating a primarily accommodative case as if it were CI, or the reverse, tends to produce the frustrating "we did vision therapy and it didn't really help" outcome — not because therapy failed, but because it was aimed at the wrong system.
